# Regulatory Daily Summary: What Happened on October 27, 2026

**FDA expands Pfizer’s prostate cancer combination, EU approves AstraZeneca asthma treatment**

The EMA committee recommended olezarsen, while the FDA issued draft guidance on some personalized gene therapies.

## Today in brief

The FDA expanded Pfizer’s Talzenna-Xtandi approval to certain adults with hormone-sensitive metastatic prostate cancer. The European Commission approved AstraZeneca’s Trixeo for asthma in eligible patients aged 12 and older. [1] [2] [3] The EMA’s drug committee recommended olezarsen for some adults with severe hypertriglyceridemia. The FDA issued draft guidance on some personalized gene therapies. [4] [5]

## FDA expands Talzenna-Xtandi to hormone-sensitive prostate cancer

The FDA expanded Pfizer’s Talzenna-Xtandi indication to adults with metastatic hormone-sensitive prostate cancer and mutations in homologous recombination repair (HRR) genes. [1] [2] Patients must have disease that is new to or still sensitive to androgen pathway treatment. An FDA-authorized test must identify an HRR mutation in one of 12 genes. [2]

In the 599-patient TALAPRO-3 trial, Talzenna-Xtandi cut the risk of radiographic progression or death by 52% versus Xtandi plus placebo. [1] [2] Median progression-free survival was not reached with Talzenna-Xtandi, versus 45.8 months with the control treatment. [2] Overall survival data remain immature. [2]

Severe anemia, the most common grade 3 or higher adverse event, affected 51% of patients who received talazoparib and 3% of controls. [2] The prescribing information warns of myelodysplastic syndrome, acute myeloid leukemia, reduced blood-cell counts and fetal harm. It calls for monthly blood-count monitoring. [2]

## EU approves Trixeo for asthma in eligible patients aged 12 and older

The European Commission authorized AstraZeneca’s Trixeo Aerosphere for ongoing asthma treatment in patients aged 12 and older whose symptoms aren’t controlled on medium-dose inhaled corticosteroid and long-acting beta2-agonist therapy. [3] The decision followed a positive opinion from the EMA’s drug committee and drew on the Phase 3 KALOS and LOGOS trials. [3]

Across the trials, Trixeo improved lung function versus the comparison treatments and reduced the annual rate of severe asthma exacerbations in a combined analysis. [3] Lung function also improved significantly within five minutes of the first dose, according to the trials. [3] Trixeo is for ongoing control, not sudden breathing problems, and doesn’t replace a rescue inhaler. [3]

## EMA committee recommends olezarsen for severe high triglycerides

The EMA’s Committee for Medicinal Products for Human Use recommended olezarsen alongside diet and other lipid-lowering treatments for adults with fasting triglycerides of at least 880 mg/dL. [4] The European Commission was expected to make a final decision in the fourth quarter of 2026. [4]

The recommendation drew on the Phase 3 CORE and CORE2 trials, which enrolled 1,063 adults and tested injections every four weeks for 12 months. [4] Olezarsen lowered fasting triglycerides by up to 72% versus placebo at six months, and the reduction lasted through month 12. [4] A pooled analysis found an 85% lower rate of acute pancreatitis events over 12 months with olezarsen than with placebo. [4]

## FDA drafts guidance for some personalized gene therapies

The FDA published draft guidance intended to streamline approval of some individualized gene therapies. [5] Rare-disease advocates have criticized the agency’s handling of these treatments. Researchers and companies see personalized therapies as a potential option for patients with rare mutations. [5]

## Also worth knowing

- The FDA approved GSK and Ionis’s bepirovirsen, marketed as Hibsago, with existing antivirals for selected adults with chronic hepatitis B who don’t have cirrhosis. Twenty-four weeks after stopping treatment, 19% of participants had viral control and undetectable hepatitis B surface antigen. The drug carries boxed warnings for serious liver reactions and low platelet counts. [6]
- China’s NMPA accepted Innovent’s application for sintilimab plus GC101 in advanced melanoma. A Phase 2 trial found median progression-free survival of 4.3 months with the combination, versus 1.6 months with chemotherapy. [7]
- Updated AHA/ASA stroke guidelines include pharyngeal electrical stimulation for swallowing problems after stroke. Chad Hoskins said guideline inclusion doesn’t automatically secure payment or coverage. [8]
- A six-month DOJ and HHS pilot lets eligible healthcare organizations disclose potential False Claims Act violations previously unknown to the federal government. Submissions are due April 22, 2027, and repayment ranges from 1.25 to 1.6 times the base repayment amount. [9]
- Proposed U.S. investment restrictions and draft Chinese export controls could affect how biotech companies structure cross-border investments, collaborations and technology transfers. [10]

## The day in numbers

- **52%**: Reduction in risk of radiographic progression or death with Talzenna-Xtandi versus Xtandi plus placebo in TALAPRO-3. [1] [2]
- **85%**: Lower acute pancreatitis event rate over 12 months with olezarsen versus placebo in a pooled analysis. [4]
- **April 22, 2027**: Deadline for submissions to the DOJ and HHS healthcare False Claims Act self-disclosure pilot. [9]
