Question 4 of 6 Intermediate Concept
What makes an effectiveness check independent and useful?
Short answer
An effectiveness check should use predeclared criteria, an adequately representative observation window and evidence evaluated independently of the person implementing the fix.
Why
For an instrument cleaning deviation, the implementer may show a new cleaning procedure and a training record. A separate reviewer should verify real cleaning records, residue or swab data where appropriate, equipment log history and repeat deviations after the change. The monitoring population must cover relevant operators, campaigns and operating conditions rather than a convenient small sample.
Design the check to detect both recurrence and unintended consequences such as longer equipment downtime or cross-contamination from a substituted solvent. Independence means critically assessing the same hypothesis and evidence, not merely a different signature. The quality system should document conflicts, decision authority and actions when results fall short.
Risk determines the depth of evidence necessary.
Common mistakes
- Using the implementer’s self-declaration as the entire independent effectiveness review
