Question 7 of 9 Intermediate Troubleshooting
How would you assess whether an undocumented manual correction affects the integrity of a batch record?
Short answer
Preserve the original record and investigate the undocumented correction as a potential data-integrity and product-impact event. Do not infer product failure or harmlessness from handwriting alone.
Why
Batch records are evidence of what was actually performed. Missing attribution, dates, reasons or obscured original entries disrupt contemporaneous reconstruction, which can conceal a dosing, timing or transcription error. Risk depends on the affected step, corroborating records and possible scope.
How to work it through
- Secure the original or validated electronic record and associated audit trails.
- Identify the correction, who made it, when and why through independent evidence such as equipment logs, dispensing records and personnel interviews.
- Assess affected batches, calculations and critical process parameters, and determine whether escalation, deviation/CAPA or batch disposition review is required under site procedures.
- Document a reconstruction with transparent uncertainty.
- Correct the process without backdating.
An example
A manual entry for material weight is overwritten without initials. The balance audit trail and second-person verification allow reconstruction of the actual dispensed quantity, but the documentation defect remains reportable and requires assessment.
Where the answer stops holding
An apparently minor change to a critical control could pose high risk. A reliable electronic audit trail does not excuse missing paper controls, and FDA guidance is nonbinding unless underlying regulations impose requirements.
A strong answer covers
- Preserve the original record and investigate the undocumented correction as a potential data-integrity and product-impact event
- Do not infer product failure or harmlessness from handwriting alone
- Secure the original or validated electronic record and associated audit trails
Common mistakes
- Backfilling signatures
- Treating a reconstructed value as if contemporaneously documented
- Automatically rejecting every batch without risk assessment
