Question 6 of 6 Intermediate Scenario

How would you manage a safety report with missing mandatory information?

Short answer

Preserve the original report, check whether the four minimum ICSR criteria are present, request missing information through permitted channels and document all follow-up. Apply reporting clocks only under the relevant rules.

Why

Imagine an email says that somebody became ill after taking a medicine, but the patient is not distinguishable and no reporter contact is provided. Register the intake, retain the unaltered information, evaluate the minimum criteria and attempt proportionate follow-up where possible. Avoid inventing a patient or assuming that missing data implies the reaction is unrelated.

If all minimum elements are available but details such as onset or dose are missing, the case may still be valid and subject to required submission procedures. Escalate any potentially serious or urgent information according to the organisation’s SOP and jurisdiction. Track when the organisation first had valid reportable information, later follow-ups and any reconciliation of linked cases.

Protect personal data. If minimum criteria never become available, retain an audit trail of the incomplete report and documented attempts rather than quietly destroying it.

Common mistakes

  • Automatically rejecting any case that lacks age, dose or laboratory values even if minimum criteria are met
