Question 1 of 6 Intermediate Scenario
How would you assess whether a CAPA was effective?
Short answer
Judge effectiveness against predeclared criteria and independent evidence after enough operating time. Closing the CAPA record proves completion, not that the cause has been controlled.
Why
Start by naming the failure mode, affected population, hypothesized cause and the control that the action is expected to strengthen. Before implementation, define a measurement method, observation window, owner and threshold for success. For example, after correcting a mislabeled sample workflow, trend label mismatches per thousand processed samples across shifts and assess related near misses.
Check that monitoring is sufficiently sensitive and that a low event count is not merely low exposure. Independent QA review should examine data integrity, unintended consequences and whether the fix works in routine conditions. If the event is rare, add a challenge test or leading indicator rather than claiming success from zero recurrences.
A recurrence may trigger further investigation, not automatic proof that every CAPA element failed.
Common mistakes
- Treating implementation signatures or a month without reported deviations as sufficient proof
